Some very young kids, like less than 8ys old, might not understand “esige” immediately another possible translation is “L’altare pretende un sacrificio”, “esige” is more formal and high level Italian and was used as the shrine is something that feels formal and powerful so an equally powerful word was used, “shrine” can also be translated as “santuario” instead of “altare”, so another possibile translation is “il santuario richiede un sacrificio”, the issue with the second one is that santuario feels somewhat heavenly and didn’t fit too well in hell and with direct sacrifices, considering today religious dont normally practice sacrifices “altrar”=“altare” felt better
